New Patients
Please complete the New Patient Forms and arrive 15 minutes prior to your scheduled visit. If you do not complete the packet in advance, you should arrive 30 minutes prior to your scheduled office visit to allow time for paperwork.
New Patient Forms Schedule an Appointment
Urology Associates will gladly file charges with your insurance company. We participate with all major carriers including Medicare and most HMO and PPO products. It is important that you provide current and accurate insurance information at each visit. Co-payments and any out-of-pocket expenses related to your visit are due at the time services are provided. Please review our Billing and Financial policies prior to your appointment date.
What to Bring to Your First Appointment
At Urology Associates we request all new patients bring the following items to ensure the best quality care is being received:
- Current List of ALL Medications
- Clinical Notes from Referring Physician or Primary Care Physician
- Photo Identification*
- Current Insurance Card
*Due to the Red Flags Clarification Act of 2010, Urology Associates is required to have an image of the patient’s photo I.D. to ensure identity theft does not occur. For more information regarding the Red Flags Clarification Act of 2010 please visit CMS.org.
